Fertility Preservation Brochure

There are two versions of the fertility preservation brochure created by the Oncofertility Consortium.  The first is the a general Consortium fertility preservation brochure.  On the left most panel under "Online Resources", physician/practice/patient navigator business cards for individual programs may be placed.  The second version is available to be customized to individual programs. Both templates, as well as an example of a finished local program brochure, are available for download.
