Evaluate the Save-My-Fertility Fact Sheets!
About our study
The Oncofertility Consortium® and the Hormone Foundation developed fact sheets for men, women, and children to learn about the impact of cancer on their fertility, hormonal management, and fertility options for them. We are evaluating these fact sheets in order to improve and expand upon them. We ask interested people to review a fact sheet in English or Spanish, and complete a short survey.
Who can join the study?
Any cancer survivor age 18-44 may participate.
What happens if you choose to participate?
Interested survivors will be asked to read a Save My Fertility fact sheet, the consent form, and complete a short survey.
How can I join?
You are being asked to participate in a survey research project entitled: “Evaluation by patients of the effectiveness of Fact Sheets developed to provide information on fertility preservation options and hormonal health after treatment,” which is being conducted by Northwestern Memorial Faculty Foundation. This anonymous survey is being done to evaluate the usefulness of the fertility preservation fact sheets found online at savemyfertility.org. No one, including the researcher, will be able to associate your responses with your identity. The survey will take about 5 minutes to complete. Your participation is voluntary. You may choose not to take the survey, to stop responding at any time, or to skip any questions that you do not want to answer. You will not have any direct benefit from taking part in this research project; however, the information learned during this study may help the researchers develop fact sheets for future patients. You will not be paid for taking part in this study. This study is being conducted by Dr. Marla Clayman and Dr. Sara Barnato Giordano. If you have questions regarding this study, please contact them at (312) 695-6180. Questions about your rights as a research subject may be directed to the Northwestern University Institutional Review Board at (312) 503-9338.
You must be at least 18 years of age to participate in this study. If you wish to participate, please click on the link and complete the survey:
